⚠️ Important: Certified copies can only be issued for births, marriages, or deaths that took place in Mantua Township.
Marriage Licenses
All couples wishing to apply for a marriage license must contact our Registrar, Pam LeVine, at (856) 468-1500 ext. 113 to schedule an appointment. Appointments are conducted on Wednesdays only.
Important: Both applicants and one witness over the age of 18 must be present at the time of the appointment.
Before your appointment, review the requirements below and gather all necessary documents.
How to Apply
All couples wishing to apply for a marriage license must contact our Registrar, Pam LeVine, at 856-468-1500 ext. 113 to schedule an appointment.
Appointments are conducted on Wednesdays only.
Important: Both applicants and one witness over the age of 18 must be present at the time of the appointment.
Before your appointment:
- Complete the Marriage License Application Form. You may download it in advance or obtain a copy at the Local Registrar’s office.
Do not sign the application before your appointment. You must sign it under oath, in the presence of the issuing authority.
Requirements for Entering Into a Marriage
To be eligible to marry in New Jersey, the following conditions must be met:
- Neither party may be in an existing civil union, domestic partnership, or marriage that is recognized by the State of New Jersey.
- Both parties must be at least 18 years of age
Where to Apply
- If either applicant resides in New Jersey, you must apply for the marriage license in the municipality where one applicant lives. The license is then valid anywhere in New Jersey.
- If neither applicant is a New Jersey resident, you must apply in the municipality where the ceremony will be performed. In this case, the license is only valid within that municipality.

Required Documents and Information
Please bring the following to your scheduled appointment:
- Valid photo identification (e.g., Driver’s License, Passport, or State/Federal ID)
- Proof of residency in Mantua Township (required for one applicant if a New Jersey resident)
- Social Security number (required by law for U.S. citizens; will be kept confidential)
- One witness, age 18 or older
- If using an online-ordained minister, a copy of their online ministry license
- $28 application fee (Cash or Check only)
⚠️ Important: Any documents written in a foreign language must be accompanied by a certified English translation.
